Associate Coaching Lab Reservation Form

These Labs are NOT open to the general public.

You must be an OFI Associate to attend. (Enrolled in OFI's A.C.T. program or Alumni)

ALL other information will be posted to GooseList (including schedule, Bridge number & pin, changes & cancellations)

Note:

1. If we do not have 4 reservations by Monday of the week BEFORE the week of the lab, that lab will be canceled.

2. You may ONLY make reservations for labs within the calendar month (so no May or June reservations in April).

3. Make sure you identify clearly which lab(s) you will be attending in the "date" and "time" fields. If we can't tell what you want, your reservation won't count.

4. If you want to make reservations for two labs in the same month, you may do that on a single form by listing both of the dates and times in the date and time fields on that form.

5. If you want to make reservations for all three labs in a single month, you may do that by typing "ALL" in the date and time fields of a single form (you don't need to list them individually).
